The Office at 7 Square

 

When calling to schedule a procedure please have the following information available:

  • Personal and insurance information.

  • Type of procedure to be scheduled.

  • Your personal medical history including any allergies, current medications, medical illnesses, or prior surgeries.

  • Name and address of your referring physician.

Cancellations

The demand for appointments is usually very high.  We therefore ask your understanding, and that you please be considerate of your fellow patients.  When it becomes necessary to cancel an already scheduled appointment, please call to cancel your appointment as soon as you know of the conflict in your schedule.  If you are unable to call during regular business hours, you may leave an after-hours message on the answering system.  This will allow other patients who are waiting for an appointment, the opportunity to be seen sooner.

There is a strict “No-Show” policy.  If you fail to appear for a scheduled appointment, and have made no effort to contact the office or your physician prior to your procedure, there will be a $100 fee charged.  Exceptional circumstances will be considered on an individual basis.
