Insurance Information

-GENERAL-

Insurance conditions today are HORRIBLE.  Both you as the patient, and we as your health care provider, are thrown into a confusing and seemingly adversarial situation with your insurance carrier.  Frequently, it seems almost impossible to obtain timely, accurate information concerning your insurance situation.  We want to develop a partnership with you, so that you totally understand your insurance policy and what it will pay.  In order to accomplish this goal we must both accept certain conditions and responsibilities.

  • It is your responsibility to supply this office with your current address, employment and insurance information, a copy of your insurance card and other forms or a referral from a doctor when necessary.

  • If you have insurance with a plan with which we participate, we will submit the claim for you.  You will be responsible for payment of any copayments, coinsurance or deductible at the them services are rendered.

  • If you have no insurance or elect to self pay, payment is due at the time service is rendered, unless other arrangements are made in advance with the insurance/billing staff. Please feel free to discuss payment arrangements with our staff.

  • We cannot guarantee your eligibility for benefits.  However, we will work with you and your insurance company to help you understand your policy, its limitations and your financial responsibility.

  • If you have Medicare, we will file the claim for you.  Once Medicare has paid the claim (hopefully in 60-90 days), it is your responsibility to pay the annual Medicare deductible and the coinsurance amount (20%), unless you have a coinsurance policy that covers this cost.

  • If you have difficulty paying, it is your responsibility to work out a flexible payment schedule with our billing staff.  We are always willing to help you and remain responsive to your financial needs.  Our goal is to help you meet your financial responsibilities without causing you financial hardship.

  • If you have any questions or concerns regarding our billing policy, please do not hesitate to ask our billing/insurance staff Remember, the health care provider concentrates only on your medical problems.  The billing/insurance staff will discuss your financial arrangements.
