 Pre-Procedure Forms
Prior to your office visit or
procedure, we mail you a welcome package with a letter introducing
you to the practice and how we conduct your visit for a procedure or
an appointment. It also includes our patient history form; also
called our kiosk.
Because these letters can get
misplaced, and because there are times when the postal service just
can't seem to find your mailbox, those documents have been placed on
this site as Adobe Acrobat files and for your convenience.

In order to help you prepare
for your upcoming procedure we have also provided you with all
the necessary forms and instructions below; including the
Patient History (Kiosk). They are here to help you so please feel free
to look at or even print them for your benefit. The are all
in Adobe Acrobat and will print perfectly on a normal 8.5 X
11 piece of paper. Orient your printer to print "Landscape"
instead of "Portrait" before you print.
